Telehealth PT Initial Evaluation
Comprehensive evaluation to individuals that are unable to physically come to in-person session.
Service Description
During your initial TELEHEALTH evaluation, your physical therapist will ask several questions about your condition, your previous level of function, and how your present condition affects your life. The initial evaluation is necessary for your PT specialist to understand your situation. At this time the therapist will establish the Plan of Care based on the findings of the evaluation, and set up goals. The treatment course is dependent on the finding at the evaluation and all subsequent visits and may include but is not limited to manual therapy, therapeutic exercises, neuromuscular re-education exercises, therapeutic activities, and or modalities Cancellation & No-Show Policy A $75 fee will be charged for missed appointments or cancellations made with less than 24 hours’ notice. By continuing with appointment scheduling, you acknowledge and agree to this policy.
